PSYCHOTHERAPY AND PSYCHOANALISES

Psychological consultig

In this site several new terms will be presented in order some information about the process of counselling to be revealed:

Psychological counseling - It is a kind of support, which is characterized with some recommendation for an action by a councilor, based on the relation between the client’s problem and the  scientifically adopted methodology , norms, aims, life strategies. (Dictionary of psychology, 1989, Sofia, p. 229)

The psychotherapist / the couselor

It is necessary to be underlined that specialists, who practice psychologal counceling, should be trained, licensed or certified, to have appropriate academic and professional education (diploma) and to practice in the field which they have been trained.

In the psychological counceling process, the counceler could meet and have conversations with the social worker, with the teacher, with representatives of other institutions. It is possible psychotherapy to be recommended or clinical psychologist consultancy, or going to physician or other type of professional care.

What does the real counseling session look like?

Normally, it takes around 50 minutes. Each session starts with a dialogue or questions around the person’s need in order the problem to be figured out.  Generally no body has no idea what will happen but the client is invited to participate in that process. Then, with the client participation, the decision is made how to proceed for further, to consult with other specialist about certain psychotherapeutically method or to continue our meetings…

What is Psychotherapy?

Psychotherapy - is an interpersonal, relational intervention used by trained psychotherapists to aid clients in problems of living (From Wikipedia, the free encyclopedia).

Psychotherapists employ a range of techniques based on experiential relationship building, dialogue, communication and behavior change.
Psychotherapy is oriented toward exploring certain psychological event, toward, conflicts, neurotic attitudes, repetition of escaping behavior its.

Psychotherapy occurs within a structured encounter between a trained therapist and client(s). Because sensitive topics are often discussed during psychotherapy, therapists are expected, and usually legally bound, to respect client or patient confidentiality.

Psychodrama as a method

Psychodrama training explores the internal world of the individual(s) through acting out their emotions and interpersonal interactions on stage.
The aim of that training is to develop creativity and spontaneity of the participants.
Psychodrama method was created by Jacob Moreno, soon after the psychoanalyses. It is almost group method but gives opportunity for working in dyads (Individual Psychotherapy). Except for Psychotherapy, Psychodrama method is useful in Education, Organizational consultancy(development) and theatre.
Besides the work with protagonist (the participant in psychodrama training), Psychodrama method gives opportunities for enrichment of “role’s repertoire” and increasing the role flexibility, spontaneously and creativity through role plays (“role training”) in a group.
Psychodrama training is very effective in situations where some blocked or unexpressed feelings could be revealed, for practicing new trends in behavior and for achieving happiness and personal growth.

WHEN IT WILL BE A GOOD IDEA TO ASK FOR THERAPEUTIC CONSULTANCY?

For children and teenagers:

From 1 to 3 years
. The child  seems non-contact, has delayed reactions, expressed no emotion.
From 3 to 6 years
Night fears, difficulty in sleeping. The child has articulation disorders, "baby voice" or  refuses to speak; exhibits hyperactivity, delays in physical development and age etc..
From 7 to 10 years
The child has difficulties in concentration, learning and mastering the material , difficulties in communication with peers and others.
From 10 to 18 years
Used alcohol and is drugs, has  behavioral and  communication difficulties, lack of peer contacts,  acts of aggression and / or self-aggression. Attempts to injure her/himself  or suicide commitment, etc.
Child Psychotherapy is a process that requires joint efforts of family, teachers and psychotherapists.. At the same time: "All of us - doctors, teachers or just parents - are convinced that children are human beings, but we   do not stop  to treat them as objects  on the pretext that their humanity will come tomorrow. We do not stop to obey factual evaluations in various forms, which could act as   crushing sentences. Our efforts to understand the child with mental challenges and help him, are often expressed in diagnosis measured in IQ or  in  the choice of teaching methods, which in fact it can often immured him  in his disability. The proofs for this is that these estimates and these measurements have repeatedly been refuted by the development of the child..”( Maud Mannoni  „ The Backward Child and His mother” р.8)

Case „R“.
May 2011
R. is 7 years child. For consultation the child and his family is directed  from a colleague of me, a psychologist, where the family was for diagnostic. The child manifests the following symptoms: difficulty in speaking, a "baby" speech, equal intonation, hyperactivity, problems in school and with peers,  diagnosed intellectual  disabilities.
After a year of psychotherapy work, along with the efforts of teachers and family, R. changes his behavior, improves the manner of speech, started to tell  stories, to participate in class work, began naming numbers,  confidently draw pictures and figures , a complete change and growth in relationships to close peers and teachers had been made.”

Case "L"
"L ' mother asked for consultation becasue of her son's problems. She said that "L" was 6 years old, frequently was ailing, especialy after  the divorce.  She pointed that the child had shared with her his fear, nightmares, especialy when he coame back home. The chld had an obssesive manner of wiping his arms and hands to the point of wounding himself. after the psycthotherapy treatment, the child stopped wiping his hands and arms, the feras and nighmares were reduced and then disapeared. For the child and his mother a new space was opened, where new situation, trauma  and conflicts were spoken interpreted.

CASES FROM THE CLINICAL PRACTICE

“Psychoanalysis specifies itself by renouncing the attempt to resolve discontent [malaise] by eradicating the symptom”.
1. "P" asked me  for psychotherapy consultation because of chronic family problems and unstable  behaviour. During the psychotherapy treatment some family models were identified which "P" unconsiously had repeated. After a period of one year and a half, "P" started to discover new manners of relationship possibilities.

2. “Mr. B turned to psychoanalysis because of a somatic symptom that medicine had been unable to treat – a hypertension that, by this point, had become so crippling that, when he came to see me ten years ago, he had not been able to work for a year. He was, he said, constantly tense, stressed, and deeply angry. This anger is a sort of stifled rage”.
In this clinical case Dominique Holvoet gradually unfolds the scenes and the crucial encounters which established the structure of the subject, as well as the formula of the fantasy. This case is about a subject that responds as a wounded man each time the Other of demand appears: “if you want me to fall, there you are!”.  Thus, he was captivated by an obsessional desire, the desire to kill desire – a death wish.

In the analytic experience “the symptom passes from its repetitive necessity … to the possibility of contingency, in other words the possibility of acknowledging the irreducible real of the symptom, offering a new trajectory for jouissance. The new status of the symptom, severed from meaning, the reconciliation with the real, opens to contingency. From this, a new responsibility is deduced for the analysand”.

The orientation of psychoanalysis is “… not to let oneself go on the side of meaning, but instead seek to make the well of meaning run dry in order get to the bone, which is the drive [pulsionnel]”.

Through this particular case, Dominique Holvoet designates the new status of the symptom, the symptomatic remainder after the revelation of the fantasy.

Family constructions and reconstructions

Yordana Hristozova - Mandradzhieva, Phd, psychotherapist, psychoanalyst in formation

Christmas time has always been associated with the family warmth and a happy family atmosphere.
In that field, an article in New York times -„The Relentlessness of Modern Parenting“, published on 25th December 2018 by Claire Cain Miller points some interesting questions to me. In it it is said that “Raising children has become significantly more time-consuming and expensive“.An example is given with a mother, who explained that her son was being enrolled in several activities as piano at the age of 5, soccer practices at the age of 4, also swimming lessons, martial arts. The author quotes the social scientist‘s comment, that „the relentlessness of modern-day parenting has a powerful motivation: economic anxiety“. And later there is explained also that „For parents, giving children the best start in life has come to mean doing everything they can to ensure that their children can climb to a higher class, or at least not fall out of the one they were born into“.
Another observation is included, made by Jessica, a sociologist at Indiana University: „Intensive parenting is a way for especially affluent white mothers to make sure their children are maintaining their advantaged position in society.“”
While these issues are based on social observations of a large number of people, the psychoanalysis deals with the complexitieson the individual level.
What does psychoanalysis say about that intensive parenting ? Is it a symptom of the family couple or a signifier of the subject?
Lacan says in „Note on the child“ that „A symptom may represent the truth of the family couple““ and later he points that „ The articulation is much more limited when the symptom that comes to dominate arises from the subjectivity of the mother. In this case the child is directly concerned as the correlate of a fantasy““.
A woman, in her mid 30-ies‘ has came for a analises. She is a mother of two kids, a girl , at the age of 6 and a babyboy. The initial demand is: to analyses herself for a professional purpose and in parallel to pay attention to her family troubles. She claims that she is getting aggressive easily to her husband as well as to her daughter.Through the analyses a family drama has been reveals , where the Jouissanceof the mother as perfect cares were not balanced by the Name of the Father and her husband potential. Something pushes the patient to act out: to cary to her daughter to the point of exhaustion, to involve herself in several facebook events and discussions, to organizes some events simultaneously. Her daughter is being enrolled in number of activities as ballet, piano lessons, musical lessons. Mother‘s idea is to „make her daughter being special, from „an upper class“.
Her professional activity is also stamped by her personality. Working with emotionally disturbed patiens without supervising becameanother field for her to manifest her symptom. Practising this job as a psychologist is her attempt to construct a solution for the subjective urgency and respectively to find a place to question the ethics of the professional standards, which are not regulated by the government.
In this case the different questions of being a daughter, a woman and a mother has been appeared urgently. The woman‘s subjectivity is frequently trying to ruin the framework of the therapy sessions, inventing different reasons. The fail of the Name of the Father in a particular society, especially in a country after post totalitarian regime, where lack of regulative mechanisms is evident in many aspects, especially in practicing psychology today, also impacts her personal constructions and phantasies.
„“The family s a birthplace of discourse, of domination“, claims Marie-Helene Brousse in her editorial words to the journal Hurly-Burly „Family dramas- family traumas, 2017“. She concludes that :“ In analysis, one learns to make do without the family in order to make use of it“.
Another young woman, in her 30-ies, soon after her second child, comes to analysis with the intensive body symptoms. Due to her analytic work, she understood that she was being raised in a family with alcohol addicted father and this impacts her choises. Her present motherhood poses question again imperatively for her to look for some solutions. In her efforts to pursue her subjective truth, she discovers that being a daughter and a woman does correlate with the role of the mother in a special way. Analyzing her personal issues and crossing her phantasm to - be a perfect mother, who is trying to involve her children in numerous events, reduces her anxiety and actings out.
In that aspect, Daniel Roy states that: „The only truth that a family can give a child is a transmission operating on the vectors of lack and name““(in Introduction to „Note on the child“Hurly-Burly, 2017)
Being touched by psychoanalysis, is a way to look for the subjective truth in the era of Fail of the Name of the Father and to face many questions the present time as well as to elaborate family reconstructions.
